You can get an application form from Customer Service Centres. Forms for the NEC for Young People are also available from secondary schools and Education HQ. Social Work offices also have forms for applicants who have a disability.
When making an application at a Customer Service Centre you must bring with you one document to confirm proof of age and one document to confirm proof of address (dated within the last 12 months).
For young people, the following are acceptable:

You will need to attach a passport size colour photograph to your application. The photograph provided needs to meet certain national standards. It should be:
- UK passport size and standard (approx 45mm high x 35mm wide)
- In colour
- Recent, in focus and recognisable as the applicant (the applicant's face must be clearly visible – no hooded top or other headwear, unless for religious purposes)
Only the applicant's face and top of their shoulders should appear in the photograph – not the whole upper body. A referee must endorse the photo and complete his/her details on the form. Please refer to the notes on the form.
If you are aged 19 to 25 (inclusive) and a full time volunteer working over 30 hours per week you are also eligible for the concessionary travel scheme - you will receive a concessionary bus card and a railcard voucher with your NEC.
For older people, the following are acceptable:

In all cases the application form will be checked then sent away and your National Entitlement Card will be sent to you through the post within approximately 14 days. The Council does not produce the card.
