By completing and submitting the form below you give permission for this child to take part in the Summer Programme organised by Dumfries and Galloway Council.

You agree that he/she will be under the authority and responsibility of the leader / coach / teacher unless accompanied by a parent or guardian.

If, at any time during the session he/she requires urgent medical treatment you give permission, provided you cannot be contacted, to the doctor or surgeon designated to make any decision necessary including administering an anaesthetic.
